Students who have taken and passed classroom drivers education at FPHS are able to request a new DEC-8 card if they have "lost" theirs.  In order for this to happen you must do the following:

1. Email Ryan Harkness at [email protected] 
2. In the email include
   1. Your LEGAL first and last name
   2. The year you took classroom drivers education
   3. The teacher you had for classroom drivers education
   4. The year you attended the 90-minute meeting (even better if you still have a picture of it that you can attach)
3. When Mr. Harkness will email you when the card has been completed and where you will be able to pick it up.  

NOTE:

1. It will take several days to complete this process as we must verify all of the information provided.  
2. Our records are final, we can not change anything from them unless there is proof or qualifications were met after the records were completed. 
3. FPHS staff can't issue DEC-8 cards for other schools or institutions. We are not legally allowed to do this.
